It is true to say that all women benefit in some way from the information and advice that can be given about the menopause. This area has become controversial with the cessation of the oestrogen plus progestogen arm of the Women's Health Initiative (WHI) in July 2002 and publication of the Million Women Study published August 2003. This care pathway is based in the Management of the Menopause 3rd edition eds Margaret Rees and David W Purdie (2002), and the British Menopause Society Council Consensus statement on Hormone Replacement Therapy (2004).

Osteoporosis

Osteoporosis affects 1 in 3 women and 1 in 12 men. Osteoporosis is defined in a National Institute of Health Consensus Statement as "a skeletal disorder characterized by compromised bone strength predisposing to an increased risk of fracture. Bone strength reflects the integration of two main features: bone density and bone quality. Bone density is expressed as grams of mineral per area or volume and in any given individual is determined by peak bone mass and amount of bone loss. Bone quality refers to architecture, turnover, damage accumulation (e.g. microfractures) and mineralization. A fracture occurs when a failure-inducing force (e.g., trauma) is applied to osteoporotic bone. Thus, osteoporosis is a significant risk factor for fracture, and a distinction between risk factors that affect bone metabolism and risk factors for fracture must be made." Fractures of the wrist, hip and vertebrae, which are the main clinical manifestations of osteoporosis, have enormous impact on quality of life, result in significant economic burden and, particularly in the case of hip fractures, are associated with considerable excess mortality. The number of hip fractures worldwide due to osteoporosis is expected to rise three-fold by the middle of the next century, from 1.7 million in 1990 to 6.3 million by 2050.
